Penile fracture – yes, the penis can break

Many men are surprised when they hear this, but it is true - the penis can break!

Even though the penis does not contain a bone, it has a strong covering around the tissue that fills with blood during an erection. When a man is fully erect, this tissue becomes tight and under pressure. If the penis bends suddenly or forcefully while erect, that outer covering can tear. Doctors call this a penile fracture, and it is a serious injury that needs immediate medical attention.

On occasions, when severe, the tear may extend to the passage that urine (pee) comes through. This is an even more significant injury and requires urgent surgical management.

How does it happen?

Penile fractures usually happen during sexual activity, especially when the penis slips out and strikes the partner's body forcefully. Certain sexual positions where the penis can bend suddenly may increase the risk. It can also happen if a man accidentally rolls onto an erect penis during sleep or tries to forcefully bend it down. In some cultures, forceful bending of the penis is sometimes practised. Although it is not very common, when it happens, it is obvious and can be very painful.

The signs to look for

Men who experience a penile fracture often report hearing or feeling a sudden popping or snapping sound. Other signs may include sudden severe pain in the penis, immediate loss of the erection, rapid swelling of the penis, bruising or dark discolouration, and the penis bending to one side. Because of the swelling and bruising, the penis may quickly look swollen and dark. Doctors sometimes refer to this appearance as the 'eggplant sign'.

This is a medical emergency

A penile fracture is not something that will heal properly on its own. Some men feel embarrassed and try to wait at home, hoping the swelling will go down. Unfortunately, delaying treatment can lead to serious long-term problems. Without proper treatment, complications may include permanent curvature of the penis, painful erections, erectile dysfunction, difficulty having sex and difficulty with urination. The best treatment is surgery to repair the tear. Of note, some men can be considered for a delayed surgical approach but this requires discussion and understanding. When surgery is done, most men recover well and can return to normal sexual function.

What should you do?

If you suspect a penile fracture, go to the nearest hospital emergency department immediately. Do not try to treat it yourself at home. Do not delay because of embarrassment. Doctors and nurses deal with these injuries professionally, and the sooner treatment begins, the better the outcome.

Protecting yourself

The most important way to reduce risk is being careful during sexual activity and avoiding sudden bending of the penis when it is erect. Understanding that this injury can happen helps men act quickly if it does occur. Penile fracture may sound shocking, but knowing the signs and seeking help immediately can prevent long-term problems.
